'Survivor: One World' Premieres With Medical Emergency

MTV News chats with 'Survivor' expert and former player Rob Cesternino about the controversial 'One World' season premiere.

The "Survivor: One World" season premiere presented longtime fans of the veteran reality series with a lot of firsts. In a brand new twist, all 18 competitors are living on the same beach from the very beginning, divided into two tribes based on gender. But that's not the only first: For the first time in "Survivor" history, the first contestant to leave the game was not voted out, but left due to a medical emergency. Kourtney Moon left the island after a nasty wrist-break in the opening immunity challenge — a challenge that the men subsequently won by default, by the way, after they were given the choice to claim victory early or continue competing against their Kourtney-less rivals.

Kourtney's medical evacuation and the short-lived immunity challenge weren't the only sources of drama. From the first minute, the male and female tribes — Manolo and Salani, respectively — made their rivalry clear through theft, fire and harsh negotiations. There is no love lost on the "One World" beach, even if there's plenty to love for the viewers at home.

For more on the "One World" premiere, MTV News joined up with well-known "Survivor" commentator and two-time contestant Rob Cesternino to cover all the blow-outs and break-ups that went down in the new season's first hour.

MTV: Rob, this first episode was a doozy. You had thievery. You had cross-tribal bargaining. Two chickens were caught. An immunity idol was found and given away. An immunity challenge was interrupted for an emergency and called early when the guys were given the chance to proceed or suspend the game in their favor. Another "Survivor" first: a medical evacuation as the very first boot. Can you remember another season premiere where so much stuff happened?

Rob Cesternino: It was such a great first two-thirds of an episode. [Laughs] It reminded me a bit of "Super 8." It was such a good start, but two-thirds of the way through, it took a turn for the worse. It was a lousy last 15 minutes of the episode. Very anticlimactic to have the first Tribal Council end with Kourtney leaving over a broken arm. This is the earliest ever med evac from the game, but not exactly the kind of history you want to make.

MTV: Maybe I'm not up to snuff on my "Survivor" rulebook, but it seems like a bit of a cheat — if Salani technically lost that challenge, shouldn't they have had to vote someone out, regardless of Kourtney's injury?

Cesternino: It's a good question. They've definitely done it where a team has lost somebody due to an emergency [and has still had to vote someone out]. But I think they felt that it would have been too much of a disadvantage for the women to be down two people already on the third day. They wanted to play out this men versus women twist. I see where they could have made them vote somebody out, but this was a little bit of a different situation. I don't think they would want to cripple their team so badly that they'd never be able to come back.

MTV: Overall, I'm pretty pleased with how the "One World" twist has played out so far. I think you get a better sense of who these people are when you can watch them engage the enemy so directly so early on. Honestly, I don't know why they wouldn't want to utilize this twist every season from here on out. But what do you think: Are you digging the two tribes, one beach setup?

Cesternino: I love the way that everything played out. Men versus women helps it in a lot of ways, too. Everyone has a rooting interest from the start. When it's green team versus red team, you're not as invested from the beginning as you are with men versus women, even if you're a man rooting for the women. Everyone picks a side early on in this situation. Also, since it's men versus women, you know right away which side everybody is on when there are 18 people living on the same beach. I really loved all of the built-in tension that came along with those guys living on the same beach.

MTV: So far, I don't think your high school alliance theory is holding up. It's very split down the middle between men versus women right now. I don't see how they're going to overcome the gender divide at this point.

Cesternino: See, that's not how I saw it. I feel like the high school alliance is playing out that way. All the young, good-looking women made their alliance, the young, good-looking men made their alliance. The only thing missing is the two alliances saying, "Hey, we have the majority if we put our two alliances together. Let's work together." Instead, you have a situation where the young women are trying to prop up the insurgents to the hot guy alliance. It's an interesting political thing that's playing out. Will the opposition to the majority alliance almost be a de facto part of the other team's majority alliance? Or could the two majorities decide they need to work together? That would be in both sides' best interest. Instead, Sabrina finds an immunity idol and gives it to Colton.

MTV: Maybe not her best move?

Cesternino: Well, considering the previews for next week she says that "Colton is a virus," I'm guessing that no, this isn't a favorable move. [Laughs] I have a lot to say about Colton.

MTV: We'll get to him in a few. But let's talk about some of the episode's other controversial moments. These teams are playing dirty against each other pretty much right away. Mike steals supplies from the girls' side, the women sneak into the men's camp in the middle of the night to steal their fire ... everyone's playing so dirty!

Cesternino: Everybody on this episode was so horrible to each other. [Laughs] Everything is so high-intensity from the get-go, but I guarantee that the person who wins this season is not going to be someone who came out so aggressively to the other team from the start. It's not a smart way to play "Survivor." At the end of the day, they'll be on the jury. If you're an ass to everybody, they won't vote for you in the end. Nobody wants to play nice, but you can't win "Survivor" by not playing nice. Well, you can win "Survivor" by not playing nice, but a lot of other things have to go right — or you have to be from Boston.

MTV: The other big controversy was the immunity challenge. Manolo was given the choice to call the challenge early and walk away with a win, or finish the challenge out and give the other tribe a shot. They chose to walk away with the win. Everyone from [host Jeff] Probst to the women was shocked. Maybe it wasn't great television, but it may have been a great move. What would you have done in that situation? Did the men make the right call?

Cesternino: Absolutely, they made the right decision. That would've been totally idiotic to give up a win in the first challenge in the spirit of sportsmanship. Could you imagine — other than some intramural sports, what other scenario would a team give up a sure win to let the other team have a shot? [Laughs] You're playing a game for a million dollars. People have to remember that this is "Survivor." Remember that they're playing for a million dollars. There was nothing mean about it. They got the win. They fought for their lives that day, and they'll see you tomorrow.

MTV: I agree — and if they'd kept going, they probably would have lost. The "Survivor" gods do not look kindly upon stupidity.

Cesternino: No. [Laughs] You don't give a win back.

MTV: Let's wrap up by looking at the players a bit. Right away, there are so many contestants that jump out and grab you, at least personality-wise if not in terms of winning the game. Who jumped out at you?

Cesternino: Colton. I'm not liking how he played the game. He's the only gay man on his tribe of all men, but I felt he really gave up from the moment they said this was going to be men versus women. No one on the men's team told him, "Hey, you're not going to fit in with us because you're gay." He decided that the men aren't going to like him, and that was it. Then you see him with the women's team, going up to every woman on the team and telling them he loves them and giving them a hug. At no point, at least they don't show it, do you see him going up to the men like that. His game is made up on day one. How does he know nobody on the men's team is going to accept him? He's already jumping ship to the women's team for no reason. He's proving himself to be very untrustworthy. That's fine if he gets along better with the women, but he's showing no loyalty to his tribe, and I think that's going to play out much more over the next 36 days of this game.

MTV: Assuming he makes it that far...

Cesternino: Assuming he makes it that far. I don't think he's come out and played a good long-term game that can be won, at least on the first episode.

MTV: On the flip side, who looks good? Who looks ready to do this?

Cesternino: I think the people who looked good are the people who shut their mouths. [Laughs] Everybody that didn't come out with vitriol towards the other tribe. You need to be a diplomat in this game. The people I liked were the people who didn't piss anybody off after only three days of being on "Survivor."

Psychotherapy May Ease Hot Flashes After Breast Cancer (HealthDay)

TUESDAY, Feb. 14 (HealthDay News) -- After breast cancer treatment, many women suffer from hot flashes and night sweats, but a type of "talk therapy" might relieve these symptoms for some women, British researchers suggest.

In a new study, women who received this form of psychotherapy, known as cognitive behavioral therapy, had reduced their symptoms by half within six months.

"Hot flashes and night sweats are distressing symptoms, which cause social embarrassment and sleep problems, and they are challenging to treat, especially for women who have had breast cancer" because hormone replacement therapy is generally not recommended for these women, explained lead researcher Myra Hunter.

According to background information in the study, which is published in the Feb. 15 online edition of The Lancet Oncology, 65 percent to 85 percent of women have hot flashes after breast cancer treatment.

Group cognitive behavioral therapy is a safe and effective treatment for women who have hot flashes and night sweats following breast cancer treatment, Hunter said, with additional benefits to mood, sleep and quality of life.

"The women in this trial reported frequent and problematic symptoms and relatively low quality of life," said Hunter, a professor of clinical health psychology at King's College London's Institute of Psychiatry.

Hunter's team randomly assigned 96 women who had been treated for breast cancer and suffered from night sweats and hot flashes to either "talk therapy" or usual care.

The 47 women who received the therapy attended weekly 90-minute sessions for six weeks. For the others, usual care consisted of access to nurses and oncologists, telephone support and cancer support services, the researchers noted.

The therapy sessions included psycho-education, paced breathing, and behavioral strategies to manage hot flashes and night sweats, as well as interactive PowerPoint presentations, group discussion, handouts and weekly homework, Hunter said.

In addition, participants learned how to handle the stress associated with hot flashes and night sweats, and found new ways to decrease anxiety, she explained.

The women were also taught to manage hot flashes in social situations and to understand night sweats and improve sleep habits using mental and behavioral strategies.

The investigators found that the women who had received the cognitive behavioral therapy significantly reduced the number of hot flashes and night sweats they experienced in the nine weeks after the start of the study.

This reduction in symptoms lasted for 26 weeks. At nine weeks there was a 46 percent reduction in symptoms and a 52 percent reduction at 26 weeks, Hunter's team found.

However, among women receiving usual care, hot flashes and night sweats decreased by 19 percent after nine weeks and 25 percent after 26 weeks.

"These reductions were sustained and associated with significant improvements in mood, sleep and quality of life," Hunter said. "This is a safe, acceptable and effective treatment option, which can be incorporated into breast cancer survivorship programs and delivered by trained breast cancer nurses."

Holly Prigerson, director of the Center for Psycho-Oncology and Palliative Care Research at the Dana-Farber Cancer Institute in Boston, wrote an accompanying journal editorial.

"Hot flashes and night sweats are very common, distressing and persistent -- women reported being troubled by them for an average of two years after breast cancer treatment," Prigerson said.

She noted that the new study provides sound evidence upon which to recommend cognitive behavioral therapy for breast cancer patients suffering from these symptoms.

"Adaptations to an online, self-management version of the intervention would allow for more flexible scheduling and greater access at potentially lower cost of delivery," Prigerson said. "Combining the intervention with medications that effectively treat hot flashes and night sweats might produce the most dramatic effects with reductions in symptoms as well as the distress caused by them."

Prigerson said this type of therapy might also be used to treat postmenopausal women suffering from these symptoms.

"Of course, scientifically, we can't generalize beyond the sample of women who experience menopausal symptoms as a result of treatment for breast cancer," she said. "But given that they found that [this type of therapy] worked on the distress associated with hot flashes and night sweats, then it would seem likely to generalize to menopausal symptoms experienced outside of this context."

Coroner seeking Houston medical, pharmacy records (AP)

LOS ANGELES ? The Los Angeles County coroner's office has issued subpoenas for medical and pharmacy records from Whitney Houston's doctors and medical providers, which is standard procedure in such investigations, an official said.

Assistant Chief Coroner Ed Winter said the request is made in virtually all death investigations because it can shed additional light on how people died and whether they had any serious medical conditions.

"We've already contacted a number of doctors with requests for records," he said.

Winter said that at this point, there is nothing unusual about how his office is proceeding with the Houston death investigation and that requests for medical records are requested through subpoenas.

"If somebody even dies in a crash, a blunt force trauma, we will still take medical issues into account," he said.

"Anything helps."

Investigators in the Houston case found several bottles of prescription medication in the Beverly Hills, Calif., hotel room where she died Saturday, although Winter has said they weren't an unusually large number. Detectives have declined to disclose which medications were seized.

Authorities said an autopsy found no indications of foul play or obvious signs of trauma on Houston. She was underwater and apparently unconscious when she was pulled from a bathtub, officials said.

It could be weeks before the coroner's office completes toxicology tests to establish the cause of death.

Medical records have become crucial in celebrity death investigations, including inquiries into what killed actor Corey Haim, actress Brittany Murphy and pop superstar Michael Jackson. Haim's and Murphy's causes of death were not drug-related, the coroner's office determined.

In Jackson's case, state and federal investigators spent months looking into Jackson's medical history and doctors who had prescribed him medication. They decided not to file charges against seven doctors who treated Jackson, although they referred one unnamed physician to the state's medical board for prescribing medications to Jackson under an alias.

Jackson's personal physician, Conrad Murray, was convicted of involuntary manslaughter for the singer's death. He had been giving the entertainer nightly doses of the anesthetic propofol in Jackson's bedroom as a sleep aid but kept no records of the treatments.

Prosecutors and experts said during Murray's trial that his decision not to keep records was reckless and deprived Jackson's family from having a full account of how he died.

Law enforcement can access California's prescription drug monitoring database known as CURES, which contains more than 100 million prescriptions and receives anywhere from 4 to 6 million additions every month. The data culled from pharmacies can determine whether doctors are prescribing outside the course of normal medical practice and see if a patient is getting multiple prescriptions from various physicians, commonly known as doctor shopping.

Gov. Jerry Brown touted the CURES program several years ago when he was attorney general, and under his leadership high-profile investigations were launched into the deaths of Jackson, Haim and Anna Nicole Smith.

In the Smith case, charges eventually were filed against two doctors and her boyfriend-lawyer in connection with her death after the database showed the former Playboy Playmate was receiving a myriad of prescription drugs. A jury acquitted the trio of most to all of the felony counts and a judge dismissed two convictions, while reducing one to a misdemeanor.

Houston died just hours before she was scheduled to perform at producer Clive Davis' pre-Grammy Awards bash. Her family plans a private church service in her hometown of Newark, N.J.

Houston, a sensation from her first, eponymous album in 1985, was one of the world's best-selling artists from the mid-1980s to the late 1990s, turning out such hits as "I Wanna Dance With Somebody," `'How Will I Know," `'The Greatest Love of All" and "I Will Always Love You." But as she struggled with drugs, her majestic voice became raspy, and she couldn't hit the high notes.

Interest in her music has skyrocketed since her death, pushing her songs back on to charts and into heavy rotation on the radio.

Lawmakers move to cusp of deal on payroll tax cut

(AP) ? Congressional leaders are gauging lawmakers' reactions to a tentative deal extending a 2 percentage-point payroll tax cut and extra jobless benefits through 2012, a pact bargainers reached after House Republicans conceded that the tax cut would not have to be paid for with spending cuts.

Both sides cautioned late Tuesday that the agreement was not final and still could be altered. If completed, final congressional approval was possible by Friday, which would be an election-year victory for President Barack Obama, who made the payroll tax cut a keystone of his largely ignored jobs creation plan in September.

House Republicans emerging from a closed-door meeting said reaction to the package was generally positive, with some saying it reflected a desire to avoid spending months debating an issue that cost them dearly last year. In December, the House GOP initially opposed a two-month extension of the tax cut and other benefits that were about to lapse, only to retreat under pressure from outside party leaders and conservatives.

"We've got to move onto another issue," said Rep. Dennis Ross, R-Fla. "I think that's what the mood is."

"I'd say most people are for it, at least accepting it," said Rep. Peter King, R-N.Y.

Republicans were determined that Obama not be able to claim that the GOP was standing in the way of a middle-class tax cut. They would rather spend the months leading up to the November presidential and congressional elections focused on GOP themes of opposing tax increases, higher spending and Obama administration regulations that they say stifle job creation.

The tentative compromise would extend through December the current 2 percentage-point cut in the usual 6.2 percent Social Security payroll tax deducted from workers' paychecks. That reduction, which saves $1,000 a year for families earning $50,000, would affect 160 million workers and would otherwise expire on March 1.

It would also prevent extra unemployment benefits for the long-term jobless from expiring March 1 and block a 27 percent cut in reimbursements for doctors who treat Medicare patients. Overall, the legislation would cost roughly $150 billion.

On Monday, House Republicans dropped their demand that the payroll tax cut ? totaling roughly two-thirds of the bill ? be paid for. The hunt for savings had been one of the chief hurdles in weeks of negotiation over the legislation.

Also excluded, aides said, was a collection of expiring tax breaks, largely for businesses buying equipment and other corporate expenses that had been sought by some lawmakers of both parties.

Participants said the Medicare payments to doctors would be paid for by reducing Medicare reimbursements to hospitals and by cutting in half an $8 billion program under Obama's health care overhaul aimed at battling obesity and smoking.

The unemployment benefits would be financed with a collection of savings that include government sales of parts of the broadcast airwaves to wireless companies and from boosting federal workers' contributions to their pensions.

Democrats were saying little publicly, and House Democrats planned to meet Wednesday to discuss the tentative plan.

In private, some Democrats called it a victory, pointing to the payroll tax cut and unemployment benefit extensions while heading off the GOP's earlier demand that the entire measure be financed with spending cuts and other savings. Others complained that the jobless benefit extensions were not generous enough.

Republicans claimed victory, too, noting their rejection of early Democratic efforts to pay for the payroll tax cut by boosting taxes on millionaires, and with jobless extensions that would be less than the 99 weeks under current law that Democrats wanted to renew.

Aides presented differing figures about how many extra weeks of coverage would be provided, but they roughly agreed that the current 99-week maximum would fall to around 73 weeks for most states. Republicans had wanted to cut the ceiling to 59 weeks.

Republicans abandoned provisions from a House-passed bill that would have required the jobless to pursue a high school equivalency degree to get benefits and let states require recipients to undergo drug testing.

They also dropped other House-passed language forcing low-income people to have Social Security numbers to get government checks by claiming the children's tax credit, a move that was aimed at illegal immigrants and caused a furor among many Hispanics.

Early Tuesday, Obama tried turning up the heat on Republicans to strike a deal.

"Just pass this middle-class tax cut. Pass the extension of unemployment insurance," he said at a White House appearance. "Do it before it's too late and I will sign it right away."
